Where are Careline Physician Services located?

We are based in over 50 counties and serve patients across Michigan, Ohio and Indiana! Click here for our service areas.

Who is eligible to receive care from Careline Physician Services?

We provide primary and specialty medical care to frail and elderly individuals with chronic and complex care needs that require higher levels of care and coordination.

What services do we provide for our patients?

Outside of regularly scheduled visits with providers, we coordinate the following services for our patients:

Home nursing services and home health aides, X-rays and diagnostic testing, medications and durable medical equipment such as walkers, wheelchairs and oxygen, specialty care referrals

Where does Careline Physician Services provide care?

We care for our patients directly from the comfort of their skilled nursing facility, home or long-term care setting.

How often will the patient receive care visits?

Care is available 24 hours a day, seven days a week, 365 days a year. The frequency and level of care varies depending on patient need/care plan. Our Advanced Primary Care team works with the patient and caregivers to create a plan of care that adapts to their needs and priorities. The average patient typically receives between 12-20 visits a year.

When is Careline Physician Services open?

Our office hours are Monday to Friday, 8am to 5pm. We have a 24-hour on-call service and are here for our patients 24 hours a day, 7 days a week and holidays.

How can I communicate with my Primary Care Provider?

Call us! We are here 24/7. We also utilize a Patient Portal to review medical visit notes, testing results, view billing statements and communication directly with the provider and our team through two-way messaging.

What happens during a typical appointment visit with the Advanced Primary Care Provider?

During an average visit with a patient, our provider will meet with the patient, talk to them about their current plan of care, review their medications, complete medical assessment and discuss the next steps in their plan of care with patient/caregivers.

How can I make an appointment?

Call us today! If you are an existing patient, you can request an appointment with your provider at any time. We have telehealth options available for urgent needs.
