1. We are always available 24 hours a day, 365 days a year, on weekends and holidays. We will help you learn about your hospice benefits and can schedule a free consultation.
2. Our Clinical Coordinator will promptly contact all responsible parties to plan consultations or patient assessments.
3. We work directly with you and healthcare professionals on a personal plan for care. We bring hospice care to where the patient calls home, whether in a private home, nursing home or other care facility.
